We serve the North Florida community as well as South Florida (Ft. Lauderdale to West Palm).
On behalf of our patients and families, we can coordinate home healthcare, private duty home care, senior living placement, labs, radiology, prescriptions, hospital care, nursing home care, telehealth visits and other needs as determined by your healthcare provider. We are now also offering management of psychiatric disorders and medications, as well as cardiac care management and coordination.

We are local to the Jacksonville and South Florida markets and easily accessible for all communication and patient primary care needs
Patients are able to keep their current doctors and specialists as desired or needed
We assist in obtaining needed insurance for home health or in-home primary care needs
We can accommodate same-day, in person or telehealth visits
Covered by insurance (Medicare, Aetna PPO, Cigna PPO, UHC PPO, Florida Blue PPO and Humana PPO)
We prevent Rehospitalization and Emergency Room visits
